The District of Columbia requires individuals under the age of 18 to obtain a work permit.

Required Documentation

  1. Copy of birth certificate or valid passport (not a copy)
  2. Copy of Social Security card
  3. Parent signed "Official Work Permit & Child Labor Application" (DC work permit)
  4. CUA supervisor supplied letter on letterhead indicating the students name, job title, what he/she will be doing, number of hours per week, and days per week, along with CUA supervisors signature.
